《病人自主權利法》於 2019 年正式上路，本文試從一位臨床醫師參與立法和末期病人各種情況的經驗，試分析此法的若干實行上的問題和提供解決方法。本文也簡要地回顧期末病人的醫療資源的開支，和說明台灣醫界習用的醫療倫理四大原則的內容。本文進而檢討在實踐上，《病人自主權利法》的困難和有待改善的條文，特別是預立意願的規定，與家屬參考的影響等。

Patient Right to Autonomy Act of Taiwan is implemented in 2019. This paper presents the point of view from a medical doctor’s experience in participation of the implementation and experience with the treatments of terminal patients. I presents briefly the medical expenses for terminal patients and the four biomedical principles prevalent among medical professional of Taiwan. This paper then shares some difficulties in its implement and suggests how to solve some of the issues, especially in the formal expression of patients’ living will, and the remaining dilemmas with the participation of patients’family in the procedure.
